This program offers Case Management Support for young people aged 12-18 within South West Sydney who are unable to live with their families.
Assistance is provided to seek or maintain accommodation, either within a family or community placement or residential setting. Young people and their carers are provided with ongoing practical, emotional and financial support.
These referrals can be on a voluntary basis or for young people in permanent care.
